Simple Web App Needed

Hi there. I need to develp a very simple web based database. Need to be able to enter entries via a browser, so a simple search to find description of the entry, and if the entry is not there, it needs to appear on the same page where people can click on it to enter a description, which then goes to me to be checked, and once I approvem, into the database so that future users can look it up. Can anyone help me to develop this? Ben
